Synthetic cannabinoids are man-made chemicals that act on the same Cannabinoid receptors as THC (delta-9-tetrahydrocannabinol) , the ingredient in marijuana. They are sprayed on shredded plant materials, packed in e-cigarettes or other devices and sold for smoking or inhalation. They are sold under different brand names such as K2, Spice, Joker, Black Mamba, Kush, and Kronic.
These synthetic cannabinoids are not known to cause bleeding. Several cases that cropped up in Illinois and other areas of the country were found to be the result of contaminated synthetic cannabinoids. These contaminants were identified to be rat poison, Brodifacoum. Brodifacoum can be separated by chromatography and identified by nuclear magnetic resonance spectroscopy. It is most widely used pesticides in the world. It was introduced in 1975 to deal with the public health problem of warfarin resistant rodents. Its anticoagulant effect is said to be 100 times greater than that of warfarin. It has a very long plasma half life. This results in a prolonged anticoagulant effect. It is often called Superwarfarin. Warfarin is used in measured doses for the management of coagulation related disorders such as deep venous thrombosis , pulmonary embolism and atrial fibrillation to help prevent clot formation. In suspected cases of adulterated synthetic cannabinoid posioning, INR as high as 20 (INR of 1.1 or below is considered normal) has been reported . Intravenous dose of Vitamin K and Fresh Frozen Plasma is required over longer period to reverse its effect.
